He compared transporting radioactive drugs to “moving melting ice cubes,” where every dose must arrive at precisely the right activity — and any disruption, including a patient missing their appointment, can waste an entire batch.

Frejd outlined how decay profiles dictate everything from manufacturing location to scheduling flexibility. Short half‑lives demand production near trial sites, while longer‑lived isotopes like actinium‑225 allow global shipping but remain in critically short supply. Consistent dosimetry, cross‑border transport rules, and uneven regulatory frameworks add further complexity.

The field now faces a strategic choice: standardised dosing that speeds trials, or personalised dosimetry that could unlock more precise and effective radiotherapeutics. “It’s a delicate balance,” Frejd said — one that will shape how widely patients can access these emerging treatments.
